Student Athlete Commitment Form

CALVARY CHRISTIAN SCHOOL STUDENT ATHLETE AGREEMENT

 

  1. Academic Eligibility Requirements

-         The student-athlete must maintain at least a 2.0 GPA

-         The student athlete must not have more than one failing grade

-         The student-athlete must not have more than one “U” mark for citizenship.

 

  1. Personal Conduct Commitment

-         The athlete will conduct themselves as worthy Christian individuals on and off the playing field.

-         The athletes must adhere to all school rules and must present themselves as a positive role model.

 

  1. Attendance Commitment

-         The athlete is expected to attend and be on time to all practices, games and other team activities.

-         Only absences for medical purposes or unexpected serious personal incidents (as determined by the school administration) will be considered excused absences.

-         Consequences can result from any absence, excused or not, and can include loss of playing time or starting position at the discretion of the head coach.

-         Unexcused absences can result in suspension or removal from the team.

 

  1. Classroom Commitment

-         The student-athlete is required to attend all of his/her regularly scheduled classes on the day of the game or practice to be eligible to participate. Absence from any class must be medically excused or pre-approved by the athletic director and /or head coach in the case of personal incidents which require the athlete’s absence from school (e.g. funerals). This is required on Fridays or Saturday activities as well. An unexcused absence from one or more classes makes you ineligible to participate in sports related activities.

 

  1. General Commitment

-         There will be a two-week trial period at the beginning of each season. At the end of those two weeks, prior to the first official game, each athlete has the option of leaving the team without penalty. Any athlete who opts to remain with the team after the trial period is committed to the team for the rest of the season. Any athlete who quits the team after this period will be ineligible for all sports for a period of one year from the date of infraction.

 

  1. High School Financial Commitment

-         To participate in interscholastic athletics for the 2009-20010 school year, we agree to pay a one time $325 “PARTICIPATION” fee due at the beginning of the first season of play.  This fee entitles my child to participate in Calvary Sports all year long.   I understand that each sport may order a “spirit pack” that would include such items as sweatshirts or warm-ups.  Those additional costs will be discussed with parents during a pre-season coaches meeting, which will occur one month before the start of the individual season.

 

  1. Jr. High Financial Commitment

-         To participate in Jr. High sports for the 2009-2010 school year, we agree to pay a one time $175 fee.   This fee entitles my child to participate in Calvary Sports all year long, and includes a PE shirt and shorts.

 

 

It is important that all CCS athletes understand the level of commitment that will be expected of them during the upcoming seasons. Your signature below, and that of your parent/guardian, acknowledges your commitment to our athletic program and is required in order for you to participate. It also represents your understanding and agreement to abide by these policies.
